Welcome Volunteers!

Below are the documents needed to apply to the volunteer program, the next volunteer orientation date, and the next CPR and 1st Aid certification class offered at our office. Please have all of the forms below completed prior to attending orientation class. You can scan and email them to renee@familylinkkids.com , fax them to 512.532.6555, or bring them completed to turn in before class.

2. Background Check Form

3. Confidentiality Statement

4. Child Abuse Acknowledgement

5. Affidavit – MUST BE NOTARIZED (most banks do this for free)

6. Life Matters! With Mariah Podcast Review  – LISTEN HERE 

Have the podcast review ready to fill out while listening.

In addition we will need:

• Cleared TB Test results with in 30 days of starting to volunteer,

• CPR & 1st Aid Certification Infant Adult (See class offered below)

• Copy of photo ID (can be made at orientation class)

 

Next Volunteer and CPR & 1st Aid Training Classes

For Foster/Adopt Parents, Volunteers, and Mentors will be:

5:30p – 6:30p Volunteer Orientation and

6:30p – 9:00p CPR and 1st Aid

In Austin: Oct 4th 2011 – 5:30p – 9:00p  - BRING PHOTO ID!!

In Austin: Nov 14th 2011 - 5:30p – 9:00p  - BRING PHOTO ID!!

In Austin: Dec 8th 2011 - 5:30p – 9:00p  - BRING PHOTO ID!!

THE AUSTIN CLASS WILL BE $25.00

payable with cash or by check (made to: CPR Resources) at the door.
