New Child Paperwork

THANKS to all of YOU who care for hurting children! Your dedication, commitment and hard work are deeply appreciated. Part of caring for children in CPS custody is strict adherence to state guidelines for documentation. Family Link constantly strives to make this process as efficient and easy as possible for foster parents. Please use the quick links below to print required documents for each child in your care.

INSTRUCTIONS:

Within 24 hours of placement, you will need to inventory all clothing and personal items brought with the child. For ages 5 and above, go over the items together and complete the appropriate Clothing Inventory Form (below), the child’s signature is required (for children 4 and younger, write “too young” on the signature line.) State requirements mandate this completed form be submitted to Family Link within 24 hours of placement.

Within 72 hours of placement, we will need the Client BrochureFamily Safety ContractConsent Form and Documentation Review.

First 21 days, complete and submit the Child’s Schedule and initial Medication Log. Medication Logs are to log all prescription AND over-the-counter medicines administered to children. Medication Logs must be faxed in monthly by the 3rd of the month for each child even if the child has taken no meds - check the “No meds prescribed this month” box. Fax forms to your Family Link Case Manager at 512-233-6473 (please call 512-233-6464 during business hours to confirm receipt of your fax.)  Create and maintain Education Binder for every school age child, print out binder instructions and dividers below. Create Medical Binder and keep copies of all medical or dental forms and all documentation from office and/or hospital visits. These binders are required to follow child to subsequent placements.  Click Medical Schedule link below to see schedule you must follow for immunizations and doctor visits.

***Regardless of income, foster parents of infants and children under the age of 5 qualify for W.I.C. – free formula and nutritious food for the children. Apply here W.I.C. Foster children are also eligible for free school breakfasts and lunches. Contact child’s school for application.***

As soon as any biological or adopted child living in your home turns age 14, please have them fill out this form Criminal Consent and send to Family Link.

As soon as any biological or adopted child living in your home turns age 18, they will have to complete an FBI Fingerprint check, click here for FBI forms.

Boys Clothing Inventory within 24 hours of placement

Infant Clothing Inventory within 24 hours of placement

Client Brochure within 72 hours of placement

Family Safety Contract within 72 hours of placement

Daily Schedule within 21 days

Medication Log for Child Over 5 within 21 days and monthly

Medical Report Take to each medical or dental visit and have office staff fax completed copy back to Family Link at 512-233-6473.

Medical Schedule follow this schedule for doctor visits, immunizations for ages 0-17, and for infant visits

Psychiatric Report To be completed by the Psychiatrist at every appointment.

Educational Binder required for all school age children – Include all report cards; testing results; and school admission, withdrawal and transfer documentation. Will be reviewed by case manager monthly.

Clothing Inventory Update use this form for all ages to document new clothing and personal items added while the child is in your care.

MOVING TO A NEW ADDRESS REQUIRED DOCUMENTS contact case manager as soon as you know you are moving to a new address. Use this checklist to obtain a Temporary Verification from case manager.
