Requirements for Foster/Adoptive Applicants:

  • At least 21 years old.
  • Married or Single. Not separated. If married, both husband and wife must complete the process to become verified and submit copy of Marriage License.
  • Minimum Income: $10,000/Single applicants – $15,000/Married Couple (add $3000 per
    child living in the home.)
  • Expenses must not exceed income.
  • Current TEXAS Drivers License and Auto Insurance with correct address.
  • Proof of homeowner’s/renter’s insurance with liability.
  • Proof of High School Diploma/GED or college transcript.
  • Must take TB test.
  • Criminal background check will be conducted for every household member 14 and above.
  • Must get CPR/First Aid Certified: Click the link to get started. Cannot be an online class.
  • Housing – of sufficient size and bedrooms to accommodate extra children; swimming pool must be secured by gated fence (speak to Family Link Foster Home Developer for specifics.)

Email 2 pictures of prospective foster/adopt parent(s) to mariah@familylinkkids.com with names of parent(s) and contact information. For married couples, please send pictures with husband and wife both in the same picture.

Below are some other forms that you can download and begin working on!

Criminal Consent – This form needs to be filled out by everyone in the household 14 years of age and older and returned as soon as possible before we can begin processing your file. It takes 48 hours for your name to get into the system before you can set up a fingerprinting appointment.

Letter Explaining FBI Fingerprinting – This form explains how to set up your fingerprinting appointment.

FAST Pass for Fingerprinting – This form is to take with you to your fingerprinting appointment.

Fire and Health Inspection Checklist – These are forms that will prepare you to get ready for the inspections your county will do for you. WE DO NOT NEED THESE FORMS TO BE FILLED OUT. They are just for you to reference. CLICK HERE FOR CITY OF AUSTIN HEALTH INSPECTION APPLICATION.

Floor Plan – This form is to help us license you for the appropriate amount of children, therefore, it is very important that you provide the dimensions of each room, including the living room, dining room and overall square footage.

Occasional Child Care Giver -  This form is to be filled out completely. The people on this list will need to fill out a Criminal Background Check (see above).

Special Needs Checklist – This form allows you to specify what needs of a child/children you feel you are and are not prepared to provide.

Pet Policy – This form needs to be filled out and returned with copies of vaccinations records (if applicable.)

Fire Escape Plan – This form is to help you prepare for a fire emergency situation. It is important that you specify the exit from each room of the home. Draw your floor plan on the page or on the back of this form.

Weapons Inventory – This form is where you can list any weapons in the home.

Affidavit – This form needs to be notarized and we need one from each parent.

Confidentiality Statement – Both parents can sign the same form.

Domestic Violence Form – This form needs to tell us how many calls of domestic violence have been reported on your home. If none, right none or NA and sign and return.

Outdoor Recreation – This form is where you can list any pools or trampolines you might have.

Outdoor Sketch – A form for sketching the layout of your yard. Include house, garage, fences, pool, outbuildings, sheds, etc…

Rules, Rewards, and Consequences – This form is where you would make a list of rules, rewards and consequences you will have in your home.

Daily Schedule – This is where you write what a typical schedule might look like.

Disaster Form – This form is where you list a place where you would go if you had ti evacuate the city.

Online Medical Consent Training – This is a form that explains how to do the Online Medical Consent Training. Make sure you print out two certificates for each adult in the home (if applicable).

Sample Menu - Fill out this form with a sample of the balanced meals that you plan to provide to foster children. This form informs you of your responsibilities for feeding foster children and communicates your understanding of these guidelines.

Transfer Form- This form is to be filled out ONLY if you are transferring from another agency, in the process of applying to another agency and have already had a home study, or have adopted previously and have a copy of your home study.
